SENIOR PARENTS:

Please print and have your son complete the Senior Player Questionnaire Form and return to the TD Club ASAP.  Info from this questionnaire will accompany your photo in the 2010 Football Game Program.  For a printable copy of this form, click HERE.

ALSO, please provide us a baby or toddler photo of your son to be included in our Senior Memories page in the game program.  Please place in the TD Club box in the school front office or mail to: MHS Touchdown Club, PO Box 3473, PTC 30269.  PLEASE BE SURE TO WRITE YOUR SON'S NAME ON BACK OF PHOTO OR PLACE IN ENVELOPE WITH HIS NAME ON IT SO THAT WE CAN RETURN THE PHOTO TO YOU.
